Since more than 85% of the U.S. population lives within 8 miles of a community pharmacy, pharmacists are often cited as the most accessible health care providers. However, in order for pharmacists to provide patient care beyond the pharmacist sector, it is necessary to establish a collaborative relationship (CPA) between the pharmacist and the physician, which specifies the specific conditions of patient care provided by the pharmacist. These patient care services may include patient evaluations, control laboratories and initiations, adaptation and/or discontinuation of current drug therapy.
